In 1975 New York City, Carlito Brigante, a seasoned career criminal, is granted an unexpected second chance at life. After serving five years of a thirty-year sentence, he is released from prison on a legal technicality, courtesy of his loyal but ethically questionable lawyer and friend, Dave Kleinfeld. Determined to leave his life of crime behind, Carlito vows to go straight, dreaming of a quiet future far from the chaos of the city.

However, Carlito’s resolve is quickly tested when his young cousin Guajiro involves him in a seemingly routine drug deal at a hidden speakeasy. The deal spirals into chaos when Guajiro’s suppliers betray him, leading to a violent shootout. Guajiro is killed, and Carlito narrowly escapes with his life, taking the $30,000 his cousin had brought to the deal. Determined to turn this blood money into a ticket out of his old life, Carlito uses it to buy a stake in a nightclub owned by Saso, a gambling addict drowning in debt. Carlito sets a goal: save $75,000 and retire to the Caribbean to start anew.

At the nightclub, Carlito encounters Benny Blanco, a brash and ambitious young gangster from the Bronx who is eager to partner with him. But Carlito, recognizing Benny’s recklessness, repeatedly rebuffs him. Meanwhile, Carlito reconnects with Gail, his former lover, a ballet dancer moonlighting as a stripper. Despite the scars of their past, they rekindle their romance, with Gail offering Carlito a glimmer of hope for the future.

As Carlito tries to steer his nightclub and his life toward respectability, trouble brews. Benny grows increasingly frustrated with Carlito’s dismissals, and their tension comes to a head during a heated confrontation at the club. Benny’s behavior escalates when he humiliates the club waitress, Steffie, in front of the crowd. Dave, spiraling into a haze of alcohol and cocaine, recklessly brandishes a gun, threatening Benny. Carlito diffuses the situation and spares Benny’s life, a choice that alienates him from his bodyguard, Pachanga, and sets a dangerous precedent.

Amid these conflicts, Carlito is reluctantly pulled back into the criminal world by Dave, who has entangled himself in a deadly feud. Having stolen $1 million from Mafia boss Tony Taglialucci, Dave is coerced into orchestrating a prison break for Tony, who is incarcerated on a prison barge. Dave begs Carlito for help, and despite his misgivings, Carlito agrees.

On a moonlit night, Carlito joins Dave and Tony’s son, Frankie, on a yacht to execute the escape. But as Tony climbs aboard, Dave impulsively kills both Tony and Frankie, fearing they would have betrayed him. Carlito, realizing the severity of Dave’s actions, cuts ties with him immediately, knowing that mob retaliation is inevitable.

The next day, Carlito learns that Dave has been brutally stabbed by a hitman but survived. Carlito visits him in the hospital, only to discover that Dave has betrayed him to the authorities, framing him as an accomplice in the Taglialucci murders. Though offered a deal by the District Attorney to save himself by testifying against Dave, Carlito refuses. He watches helplessly as the cycle of violence closes in on his old friend, who is gunned down by Tony’s surviving son, Vinnie, disguised as a police officer.

Determined to escape the mounting chaos, Carlito plans a fresh start with Gail, now pregnant with his child. He buys train tickets to Miami and retrieves the stashed money from his club. But when he arrives, a group of Italian gangsters led by Vinnie ambush him. Carlito navigates the labyrinthine streets of New York, evading his pursuers through the subway and into Grand Central Terminal. A fierce gunfight ensues, leaving Carlito as the last man standing—until Vinnie is killed by the police.

Racing against time, Carlito runs to board the train where Gail and Pachanga are waiting. But just as the promise of freedom seems within reach, Benny Blanco emerges from the shadows and ambushes him. Armed with a silenced pistol, Benny fatally shoots Carlito multiple times. In a chilling betrayal, Pachanga confesses he has sided with Benny, but Benny silences him as well.

As life slips away, Carlito hands Gail the money, urging her to leave and build a better future for their child. Collapsing on the station floor, Carlito’s vision blurs as he gazes at a nearby billboard of a serene Caribbean beach. The image comes alive in his mind, transforming into a radiant vision of Gail dancing in the tropical paradise they had dreamed of. With that final dream, Carlito’s story ends, his hope for redemption carried forward in Gail and their unborn child.
