
Premonition and Highway Disaster
A year after the tragic explosion of Flight 180, college student Kimberly Corman embarks on a road trip to Daytona Beach, Florida, for spring break with her close friends Shaina McKlank, Dano Estevez, and Frankie Whitman. As they approach the entrance ramp to U.S. Route 23, Kimberly is struck by an unsettling premonition—a massive and deadly highway pile-up triggered by a logging truck. In an attempt to prevent the inevitable, she stalls her car at the entrance, inadvertently blocking multiple vehicles from entering the highway. Among those stopped are lottery winner Evan Lewis, mother Nora Carpenter and her teenage son Tim, businesswoman Kat Jennings, stoner Rory Peters, high school teacher Eugene Dix, pregnant Isabella Hudson, and state trooper Thomas Burke. While Burke confronts Kimberly, her vision unfolds in real time, as the catastrophic accident claims numerous lives, including Shaina, Dano, and Frankie. At the last second, Burke pulls Kimberly from her vehicle, saving her just before another car slams into hers.
The Survivors and the First Deaths
Following the incident, the survivors are questioned at the police station, each of them struggling to make sense of the inexplicable event. However, what seems like fate takes a sinister turn when Evan is gruesomely impaled by a fire escape ladder after narrowly escaping a fire in his apartment. Realizing that something unnatural is at play, Kimberly seeks out Clear Rivers, the last known survivor of Flight 180. Clear, having confined herself to a psychiatric ward for protection following the death of Alex Browning by a falling brick, is reluctant but ultimately agrees to help. Upon hearing that Evan was the first among the highway survivors to die, despite not being first in Kimberly’s premonition, Clear deduces that Death is working in reverse order.
Meanwhile, another horrific accident occurs—Tim, after a routine visit to the dentist, is suddenly crushed by a massive falling windowpane, further confirming that the survivors are marked for death. Desperate for answers, Clear introduces Kimberly and Burke to William Bludworth, a mysterious mortician who cryptically explains that only “new life” can break Death’s cycle. Convinced that Isabella’s unborn child is the key to survival, Burke arranges for Isabella to be taken into protective custody by fellow marshal Steve Adams. Meanwhile, the remaining survivors take shelter in Burke’s apartment, hoping to avoid Death’s design. However, when Nora decides to leave, she falls victim to a tragic and gruesome accident, being decapitated by an elevator in a chilling sequence of events.
The Pursuit of Isabella and More Tragedy
Determined to prevent more deaths, the remaining survivors set out in Kat’s SUV to find Isabella. During the pursuit, Isabella goes into labor, prompting Adams to rush her to the hospital. Along the way, Kimberly pieces together a terrifying realization—the deaths of the Flight 180 survivors had already altered the fates of everyone involved in the highway disaster, unknowingly saving them from prior, unrelated deaths. This revelation inverts the sequence of Death’s design, making them more vulnerable than ever. Before they can act on this information, Kat’s SUV suffers a tire blowout, causing a deadly crash into a farm’s PVC pipe storage. Though the initial impact doesn’t immediately kill anyone, Eugene is severely injured, and Kat is trapped in her seat. As rescue workers attempt to free her, a freak accident with the vehicle’s airbag causes Kat to be impaled by a metal pipe. Moments later, her discarded cigarette ignites a fuel leak from a nearby news van, triggering a massive explosion. The blast hurls a barbed wire fence into the air, slicing Rory into pieces in a gruesome spectacle.
The Final Confrontation with Death
Guided by a final vision of a doctor named Kalarjian, whom Kimberly believes is meant to euthanize Isabella, she, Clear, and Burke rush to the hospital in a desperate bid to stop another tragedy. However, upon arrival, they realize that Isabella was never meant to die in the pile-up at all—her survival had always been ensured. At the same time, Eugene’s hospital ward suffers a devastating explosion due to an oxygen leak, killing both him and Clear in an instant. Kimberly, now the only person who fully understands the cycle, has an epiphany—new life doesn’t just mean birth; it means rebirth. In a last-ditch effort to defy Death, she deliberately crashes her car into a lake, intending to drown herself. Burke arrives just in time to pull her from the wreckage, and Dr. Kalarjian successfully revives her. This act of resuscitation effectively creates “new life,” seemingly ending Death’s pursuit.
A False Sense of Security
In the aftermath, Kimberly and Burke attempt to return to normalcy, joining the Gibbons family for a lakeside picnic. As they reflect on their survival, the Gibbons reveal that their son Brian was nearly hit by a news van on the day of the accident but was saved by Rory. Just as Kimberly and Burke process this realization, Brian is suddenly killed in a freak explosion caused by a malfunctioning barbecue grill. The shocking event serves as a grim reminder that Death’s design can never truly be escaped.
