In 1975, in the Irish-American neighborhood of Charlestown, Boston, young friends Jimmy Markum, Sean Devine, and Dave Boyle are playing street hockey. After inscribing their names into a patch of wet concrete on the sidewalk, two men posing as police officers approach them. These men abduct Dave and hold him captive for four days, subjecting him to horrifying sexual abuse before he finally escapes.

Fast-forward twenty-five years: Jimmy, once a convict, now runs a neighborhood convenience store. Sean has become a detective with the Massachusetts State Police, although his personal life is in shambles after his pregnant wife, Lauren, left him. Dave, still haunted by the trauma of his abduction, works as a blue-collar laborer and struggles to move past the scars of his past. Their lives remain intertwined through marriage—Dave’s wife Celeste is a cousin to Jimmy’s second wife, Annabeth.While Jimmy grieves the loss of his daughter and embarks on a personal quest for answers, Sean and his partner, Whitey Powers, take on the official investigation. A witness statement suggests that Katie may have been acquainted with her killer. Their probe reveals that the murder weapon, a .38 Special revolver, was also used in a 1984 liquor store robbery committed by Brendan’s father, “Just Ray” Harris. Though Ray has been missing since 1989, Brendan insists his father still sends the family $500 every month. When questioned, Brendan denies any knowledge about the gun.

Suspicion begins to circle around Dave, who keeps altering his account of how he sustained his injuries that night. His erratic behavior deeply unsettles Celeste, who grows convinced that Dave may have been involved in Katie’s murder. She confides her suspicions to Jimmy, further igniting his anger and resolve to find justice for his daughter.

Jimmy, alongside his old friends, invites Dave to a bar. They ply him with alcohol, and when Dave becomes visibly sick, Jimmy confronts him. During their heated exchange, Jimmy confesses that he killed “Just Ray” years ago for implicating him in the liquor store robbery—a betrayal that led to Jimmy’s imprisonment. Dave, in turn, admits that he did kill someone the night Katie was murdered, but it was not her. He claims to have beaten a child molester to death after finding him with a child prostitute. Unconvinced by Dave’s story, Jimmy pulls out a knife and demands a confession. Terrified, Dave falsely admits to Katie’s murder, and Jimmy kills him. He disposes of Dave’s body in the Mystic River, ending the confrontation with a chilling finality.

Meanwhile, Sean and Whitey piece together the truth about Katie’s murder. Discovering that Brendan’s younger brother, “Silent Ray,” and his friend John O’Shea are responsible, they rush to Brendan’s house. By the time they arrive, Brendan has already confronted the pair after noticing his father’s gun was missing. In a fit of rage, Brendan beats both boys, demanding they admit their guilt. John, panicking, pulls out the revolver and prepares to shoot Brendan, but Sean and Whitey arrive just in time to disarm him and arrest both teens. It is revealed that Katie’s death was the result of a prank gone horribly wrong.

The following morning, Sean informs Jimmy that the real culprits—John and “Silent Ray”—have confessed to the crime. He also tells Jimmy that Dave is now a suspect in the murder of the child molester but has not been found. Jimmy says nothing about Dave’s fate, instead thanking Sean for catching Katie’s killers. However, he bitterly remarks that things might have been different “if only you’d been a little faster.” Sean pointedly asks Jimmy if he plans to send Celeste $500 each month, mirroring the payments “Just Ray” had supposedly sent his family.

Sean reconciles with his estranged wife, apologizing for pushing her away, while Jimmy confesses to Annabeth about killing Dave. Annabeth reassures him, declaring that he is a “king,” a man who does what needs to be done, no matter how difficult. Her unwavering support contrasts sharply with Celeste’s betrayal of her husband. Annabeth mocks Celeste, accusing her of speaking ill of Dave and abandoning him when he needed her most.

During a neighborhood parade, Dave’s son, Michael, waits in vain for his father to return. Sean spots Jimmy among the crowd and makes a gun gesture with his hand, silently signaling that their conflict is far from over. Jimmy, unfazed, spreads his arms in a defiant gesture, daring Sean to do what he must. The shadow of violence and unresolved guilt lingers over Charlestown, leaving the bonds of loyalty and betrayal forever intertwined in the lives of those who remain.
