The story begins in the grim confines of a Salvadoran prison, where Jack Conrad (Steve Austin) awaits execution. His days are numbered until Ian Breckel (Robert Mammone), a wealthy and unscrupulous television producer, intervenes, “purchasing” Conrad and nine other condemned criminals from around the globe. Breckel transports them to a remote South Pacific island, where they are offered a grim bargain: fight to the death in a broadcasted survival game. The last one standing will win freedom and a cash prize, but failure means certain death.
The contestants are fitted with ankle bombs, each equipped with a 30-hour countdown, a tracking device, and a failsafe pin that can trigger the explosive with a ten-second delay. Breckel, obsessed with creating a spectacle, aims for online ratings rivaling the Super Bowl’s viewership, enticing audiences with raw violence and human desperation.
As the game begins, chaos erupts. The contestants are dropped off across the island, but tragedy strikes immediately when Dominic Giangrasso, an Italian convict, falls through a rotting pier and dies impaled on a wooden beam. Alliances and rivalries form quickly. Ewan McStarley, a sadistic Irishman, teams up with Saiga, a cold and calculating killer, to eliminate the competition systematically. Meanwhile, Yasantwa, a cunning and manipulative participant, uses her wiles to lure Bruggerman and Mackie into fatal traps.
Jack Conrad, quiet but calculating, works alone until he crosses paths with Paco, a desperate man searching for his wife, Rosa, who had been abducted by Breckel’s men. The two form a fragile alliance. Conrad manages to kill one of the competitors, Raudsep, but their partnership takes a devastating turn when Paco reunites with Rosa, only to see her brutally assaulted and murdered by McStarley and Saiga. Injured and broken, Paco dies soon after, leaving Conrad to seek vengeance.
The game’s brutality begins to wear on Breckel’s team. Julie, his girlfriend, and Goldman, the production manager, start voicing their disgust over the escalating violence. However, Breckel dismisses their concerns, blinded by his greed and lust for ratings.
Conrad, now driven by rage, recalls seeing the game’s broadcast tower earlier and infiltrates it. He manages to contact his girlfriend, Sarah, back in Texas, providing her with the latitude of the island before security head Baxter chases him off. Sarah, devastated by the discovery of Jack’s involvement in the game, watches the live broadcast in horror, determined to find help.
Back on the island, McStarley and Saiga’s alliance fractures when they encounter Yasantwa. In a desperate move, she detonates her bomb, killing herself and injuring Saiga. Conrad soon finds himself facing the two remaining competitors. In a tense battle, Conrad kills Saiga with a knife, forcing McStarley to flee. The producers, eager for a climactic finale, drop a shotgun to McStarley, who uses it to hunt Conrad relentlessly.
The showdown between McStarley and Conrad reaches a breaking point when McStarley ambushes Conrad near a stream. In a desperate maneuver, Conrad rolls off a cliff and into the water below. Presumed dead by both McStarley and the production team, McStarley is declared the winner and transported to the control tower to collect his prize. But Breckel has no intention of keeping his promises. After deactivating McStarley’s bomb, Breckel reveals that the promised cash and freedom were never part of the plan.
Meanwhile, Conrad resurfaces, alive and determined to end Breckel’s twisted game. He confronts Baxter at the control tower, killing him in a brutal fight, before turning his attention to Breckel. Inside the building, McStarley, enraged by Breckel’s betrayal, takes an MP5 submachine gun from a guard and begins killing members of the production team. Although they were unaware of Breckel’s deceit, McStarley shows no mercy. When he corners Julie, he is interrupted by Conrad.
The two men face off in a final confrontation. Conrad shoots McStarley multiple times, but not before acknowledging the pain and torment McStarley endured in prison, which drove him to such savagery. With McStarley dead, Conrad seizes his bomb, now reactivated by Julie, and pursues Breckel, who is attempting to flee in a helicopter.
In a dramatic climax, Conrad hurls the bomb into the helicopter. Breckel, desperate to save himself, grabs for it, but the device explodes. The helicopter erupts in flames, crashing into a nearby cliff and killing Breckel. The destruction marks the end of the deadly game.
With his own bomb deactivated, Conrad is finally free. He returns to Texas, where Sarah, believing he had died during his confrontation with McStarley, is overwhelmed with relief to see him alive. Reunited, they embrace, leaving the horrors of the island behind as they begin a new chapter, free from the grip of Breckel’s twisted creation.
